Late Monday afternoon, May 13th, Oregon State Police responded to a three vehicle crash on Hwy-101, north of Bandon, in Coos County just after 4:30pm..
The preliminary investigation indicated a northbound Nissan Frontier, operated by 63 year old Tracy Martin Goforth of Gold Beach, crossed into the southbound lane for unknown reasons, striking a Toyota Prius, operated by 76 year old Ronald Willam Lyons of Bandon, head-on.
The Nissan came to rest in the northbound lane while the Prius spun in the southbound lane striking a Toyota Venza, operated by 70 year old Dennis Joseph Dugan of Bandon, nearly head-on.
The operator of the Nissan, 63 year old Tracy Martin Goforth of Gold Beach, was transported and declared deceased at the hospital.
The operator of the Prius, 76 year old Ronald Willam Lyons of Bandon, and passenger, 73 year old Delia Villarreal Lyons of Bandon, were transported due to injuries suffered during the crash.
The operator of the Venza, 70 year old Dennis Joseph Dugan, and passenger, 69 year old Mary Therese Dugan of Bandon, were transported due to injuries suffered during the crash.
The highway was impacted for approximately 15 hours during the on-scene investigation and the cause of the crash is under investigation.
